What you’ll do 💙
✔️ Assess patients for home infusion therapy ✔️ Coordinate care plans, supplies, and delivery schedules ✔️ Partner with home health agencies, referral sources, and pharmacy teams ✔️ Support patient and caregiver education ✔️ Monitor progress, therapy compliance, and care needs ✔️ Keep clinical documentation accurate and timely
What you’ll bring 🌟
✅ RN license in the state of practice ✅ Associate’s degree required, Bachelor's preferred (BSN). ✅ At least 1 year of nursing experience in home health or infusion therapy is preferred or 2-5 years of nursing experience ✅ Knowledge of infusion techniques, home care protocols, and patient education ✅ Strong communication, organization, and patient-first mindset
