The Social Worker is responsible for conducting psychosocial evaluations and providing ongoing counsel and bereavement services to patients and families. They also manage community education outreach and provide emotional and spiritual support in accordance with the plan of care.
Candidates must hold an MS degree in social work from an accredited school and possess a current state license as a social worker. A minimum of 2 years of clinical experience, preferably in healthcare or hospice, and reliable transportation are required.
