The Facilitator engages high-need youth and families to develop and monitor individualized care plans using the High-Fidelity Wraparound model. They are responsible for building natural support systems and maintaining strong relationships with community partners.
A Bachelor's degree in human services or a related field is required, though experience may substitute. Candidates must have two years of experience providing services to children, youth, and families.
