Responsible for assessing and planning patient care in collaboration with other home care disciplines. Provides social work services from admission through discharge and assists clients in accessing community, state, or federal services.
Requires a Bachelor's degree in Social Work, a current Kansas Social Worker License, and at least one year of healthcare social work experience. Candidates must have a valid driver's license, auto insurance, and pass required criminal record and safety registry checks.

Bachelor's degree in Social Work. Current Kansas Social Worker License Minimum 1 year Social Work experience in a Health Care setting. Auto liability insurance. Clean driving record.
