About Us - Home Care Freeman Home Care's mission is to improve the health of the communities we serve through contemporary, innovative and quality healthcare solutions in the home care setting Our team of professionals shares common values, vision and purpose We work closely with patients, families and physicians to develop individualized care plans to best meet the needs of each patient Services include: Skilled nursing, skilled therapy, medical social work and home health aide services Schedule: Full-time What You’ll Do Responsible for assessment and planning patient care in conjunction with other disciplines in home care. Provides social work service up to and including discharge for patients requiring those services. Assists client in obtaining available community, State or Federal services.

Requirements

Bachelor's degree in Social Work. Current Kansas Social Worker License Minimum 1 year Social Work experience in a Health Care setting. Auto liability insurance. Clean driving record. Current driver's license. Must be able to successfully pass KDADS criminal record check upon hire. Must register with and successfully pass the Family Care Safety Registry upon hire.
