Conduct community outreach to identify individuals in need of primary health care and provide information on available services and insurance options. Coordinate health fairs, perform patient screenings, and provide medical interpretation and transportation assistance for high-need patients.
A high school diploma is required, while an associate's degree is preferred. Candidates must possess strong interpersonal skills to build relationships with community members and migrant workers.
The Community Outreach Worker will conduct outreach and provide supportive services for potential and current patient in the community. The position requires strong interpersonal skills and the ability to make connections and build relationships with community members. The Community Outreach Worker will work collaboratively with other members of the outreach team and community-based agencies to identify individuals in need of preventative and primary health care services. The Community Outreach Worker will provide information about the services and programs available; health insurance/ coverage options and linkage to additional health care services and resources as needed.
