Provide in-home physical therapy visits focusing on fall prevention and mobility for seniors. Perform evaluations, reassessments, and maintain accurate documentation of patient progress.
Must hold a current Hawaii license as a PT or PTA with at least one year of clinical experience. A valid driver's license and reliable transportation are required.
