Bay State Community Services is seeking a compassionate, organized, and mission-driven Intensive Care Coordinator (ICC) to join our Plymouth Community Service Agency (CSA). The ICC serves as the primary point of coordination for youth with Serious Emotional Disturbance (SED) and their families. Using the Wraparound model, you will bring together providers, natural supports, and community resources to build a comprehensive care plan that reflects the family’s strengths, needs, and goals. This is a dynamic, relationship-focused position ideal for someone who excels at communication, problem-solving, and advocacy.

Bachelor’s degree in Human Service field and minimum 1 year relevant experience in navigating the child/family serving systems and experience advocating for family members who are involved with behavioral health systems; OR Bachelor’s degree in non Human Service field and more than 1 year relevant experience in navigating the child/family serving systems and experience advocating for family members who are involved with behavioral health systems; OR Associate’s degree or High School Diploma and minimum of 5 years relevant experience in navigating the child/family serving systems and experience advocating for family members who are involved with behavioral health systems Master's Degree in a Mental Health Field including but not restricted to Counseling, Family Therapy, Social Work, Psychology etc. from an accredited college/university Willing to become certified in MA Child And Adolescent Needs and Strengths (CANS), program will provide training.
