The Care Coordinator provides complex administrative and clinical support for Maternal-Fetal Medicine patients to ensure timely access to care. Key duties include acting as a communication link between patients and staff, coordinating visits, and supporting chronic disease management programs.
Candidates must be graduates of an accredited Medical Assistant program with a current Washington State MA-Certified credential and BLS certification. At least two years of relevant clinical experience is required, with a preference for outpatient and chronic disease care.
The Vancouver Clinic is hiring a Care Coordinator supporting Maternal-Fetal Medicine, providing complex and diversified support to the care team by coordinating care for patients on the daily schedule. This role requires advanced administrative skills and a team-based approach, working in continuous partnership with patients and providers to promote timely access to care, continuity, and patient well-being. Responsibilities include serving as a communication link between patients and clinic staff, identifying care needs through reports, evaluating patient records, assisting with visit coordination, supporting chronic disease management programs, and providing support to the referral specialist.
Graduate of an accredited Medical Assistant program. Current Washington State Medical Assistant-Certified credential required. Certification from national certifying entity (AAMA, NCCT, AMT, NHA, etc) as required for MA-Certified credential. Current healthcare provider BLS. At least two years relevant experience in a clinical setting required, outpatient care and service of patients with chronic disease preferred.
