RN Care Managers facilitate patient progress through the continuum of care in an acute care setting. Key duties include transition planning, care coordination, community referrals, and monitoring plan effectiveness.
Requires a nursing degree, Oregon RN license, and at least 2 years of acute care or community-based experience. Candidates must have experience with electronic medical records and specific care management or discharge planning experience.

RN Care Managers, in the acute care setting, provide a variety of services to appropriately help progress patients through the continuum of care. Key services include screening for post-acute needs; transition planning; care coordination; education; community care referrals; care planning in accordance with regulatory requirements; collaboration with the continuum care team; ongoing monitoring of plan effectiveness and reassessment of the need for Care Management (CM) services.

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
