Social Worker 2 - Adult ER: 3rd Shift

We currently offer an opportunity for an outpatient, Full Time, 3rd shift, Social Worker to support adult emergency room patients in our Level 1 Trauma Center in Cleveland. Serves as a resource within the social work department and the hospital. What You Will Do Provides psychosocial assessment / intervention to assist patients/families (including clinically complex).
• Provides age specific care to patients/families in all age groups. Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs.
• Develops plans with attention to individual patient and family goals
• Provides linkage to community resources and UH network services to facilitate optimal care; and serves as liaison to community agencies. Communicates/collaborates with the work team using a multidisciplinary approach. Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers. Assists with recruitment, and orientation/mentoring/education of new staff and/or social work interns. Additional Responsibilities Performs other duties as assigned. Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.

Education Master's Degree from an accredited school of Social Work (Required) Work Experience Hospital/Health Agency experience. Position Details

Previous social work experience is strongly preferred. Prior work in an emergency room or urgent care is also preferred. Schedule is: Monday-Friday with rotating weekends and holidays. This position is full-time, 3rd shift (8:00pm start time) in the Adult Emergency Department. The schedule can be (3) 12’s plus (1) 8 hr. (every other week), OR (4) 10’s.
