Provide speech-language therapy and wellness services to older adults within an integrated care model. Responsibilities include conducting assessments, creating personalized care plans, and coordinating care through wellness check-ins and caregiver communication.
Candidates must hold a current professional license in speech-language pathology (SLP). Strong clinical skills and a collaborative spirit for working with seniors in a community setting are required.
