The Care Coordination RN performs discharge planning evaluations and risk assessments to ensure appropriate patient transitions. They collaborate with interdisciplinary teams and social workers to coordinate complex home care plans and educate patients and families.
Candidates must be a Licensed Registered Nurse in New Jersey with 3 to 5 years of acute care nursing experience. A BSN is preferred or must be obtained within three years of hire.

In addition to base salary, some positions may be eligible for clinical certification pay and shift differentials. The salary range listed for exempt positions reflects full-time compensation and will be prorated based on employment status.
