The coordinator evaluates acute and chronic health conditions for palliative care patients through interviews, vital signs, and specimen collection. They also manage administrative tasks such as medication refills, prior authorizations, and coordinating telehealth appointments.
Candidates must be a Medical Assistant, Patient Care Technician, Health Coach, or CNA with at least 3 years of related home health or clinical experience. Proficiency in Word processing and internet software is required, with Athena EHR experience preferred.
In collaboration with the Nurse Manager, Palliative and Complex Care and oversight of the Palliative and Complex Care Advanced Practice Provider and/or Primary Care Provider, the Palliative and Complex Care Coordinator expands the individualized primary healthcare services by evaluating certain acute or chronic health conditions for our Palliative Care organization.
