The Per Diem Home Care Nurse provides assessments, care plans, recommendations, and revaluations to clients in their homes on an as-needed basis. Essential responsibilities include conducting in-home patient assessments, developing individualized plans of care, and educating clients and caregivers on disease management and safety.
Candidates must possess an active RN license, current BLS/CPR certification, and a minimum of 1 year of nursing experience, with home health or hospice experience preferred. A valid driver's license and reliable transportation are mandatory, along with strong clinical assessment and critical thinking skills.

We are hiring a Per Diem Registered Nurse (RN) to conduct in-home client assessments and develop individualized care plans for private pay clients.
You create your own schedule based on availability.
This is a non-Medicare, non-OASIS role focused on clinical assessment and care planning.
Participation in training sessions is completely optional and can be scheduled based on your availability and interest.
