Acts as a Clinical Care Coordinator for outpatient Hematology/Oncology patients, providing education and symptom support. Responsibilities include coordinating infusion scheduling, conducting patient visits, and collaborating with medical providers for continuity of care.
Requires a current Montana RN license or multi-state compact license and RQI Provider (BLS) certification. A BSN degree and at least two years of nursing experience, preferably in oncology, are preferred.
Current Montana RN License or a multi-state compact license with authorization to practice nursing within the state of Montana; required. BSN degree preferred. Oncology related experience is preferred, but not required. RQI Provider (BLS) within seven (7) days of hire/transfer or before independent patient care, whichever comes first, required. Minimum of two (2) years’ nursing experience preferred. Promotes excellence through setting high standards and providing high quality outcomes. Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently. Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy. Possess and maintain computer skills to include working knowledge of Microsoft Office Suite and ability to learn other software as needed. Job Specific Duties: Performs patient assessment. Utilizes all available sources of information and incorporates the patient’s perception of illness. Develops an individualized plan of care for each patient and updates the plan throughout the visit as appropriate. Consistently identifies safety issues concerning the patient and staff during visit. Independently reviews medication lists and provides ongoing medication or treatment education at the directive of physician or advanced practice provider. Coordinates with healthcare team to provide continuity of care. Participates in the development and delivery of education and competency validation of clinical staff. Utilizes department Standard of Care and individualizes care provided on an on-going basis per department standards. Administers medications per provider order. Demonstrates a commitment to professionalism while undertaking and completing all department competencies.
