The Case Planner manages case intake and provides essential service needs for families in accordance with ACS standards. Responsibilities include conducting weekly home visits, providing counseling, and coordinating social service referrals.
A Bachelor's degree in Social Work or a related field is preferred, along with 2-3 years of experience in child welfare. Candidates should be bilingual (preferably Spanish or French/Creole) and proficient in Microsoft applications.
Under the supervision of the Supervisor, the Case Planner will be responsible for case intake, management of cases and providing service needs, in accordance with ACS standards and indicators and Rising Ground policy and procedure manual.

Full time position with office location 111 Livingston Street Brooklyn Hub, scheduled appropriate to the needs of client and families. This position calls for a thirty-five (35) hour work week, but more than 35 hours per week may from time to time be required.
