Responsible for all aspects of pharmaceutical care, including evaluating medication orders and supervising the accurate dispensing of drugs. Provides clinical services such as renal dosing and anticoagulation monitoring for patients across all age groups.
Requires a degree from an accredited College of Pharmacy and current licensure as a Registered Pharmacist in Iowa. 1-3 years of job-related experience is required, with a Pharm.D. and residency preferred.
Verifies all orders generated on shift, keeps work flow up to date and current, and cleans up old orders as soon as possible. Completes verbal hand off communication to next shift, to ensure continuity of quality patient care and follow through in interventions. Screens and affirms appropriateness of orders including allergy check, formulary compliance, pregnancy & lactation status, dosing, duplicate orders etc. Documents adverse drugs reactions & medication errors in the RL system as they occur throughout the medical center. Fields and investigates drug information questions. Uses resources appropriately and colleagues as needed to ensure timely and accurate responses. Documents as necessary. Checks and assures high quality of IV Room products every time. Ensures timeliness of delivery of final product, utilizing means of maximum efficiency. Enters TPN orders into compounder computer system accurately and timely.
Education: Graduate of accredited College of Pharmacy required. Pharm.D. Residency preferred.
