Provide quality medical care within a Patient-Centered Medical Home team to improve patient access and health outcomes. Responsibilities include participating in team huddles, maintaining accurate patient records, and collaborating with healthcare professionals.
Must hold an active Ohio license, a Certificate of Authority in the applicable field, and current CPR certification. Experience with medically indigent populations and bilingual proficiency in English and Spanish are highly desired.
Monday – Friday schedule. No late nights, no weekends, and 11 paid holidays.
Active Ohio License: Must hold a current and valid license in the State of Ohio. Certificate of Authority: Required in the applicable field of practice. CPR Certification: Current CPR certification is mandatory. Language Skills: Bilingual proficiency in English and Spanish is preferred.
