Provide dignified personal care and emotional support to terminally ill patients within their homes. Monitor and record vital signs while reporting patient changes to the interdisciplinary care team.
Requires an active CNA license and current CPR certification. Preference is given to candidates with over one year of experience in hospice or home health settings.
What You'll Do As a Transitions Hospice STNA, you're a vital member of the interdisciplinary care team — not just a support role, but a trusted presence for patients and families. What You Bring Active STNA License or Certification in the state of employment Possess and maintains current CPR certification One (1) + year Hospice, home health or related organization experience preferred Comfort and sensitivity working with terminally ill patients and their families A calm, dependable presence under emotionally demanding circumstances Strong attention to detail in observation and documentation A valid driver's license and willingness to travel locally for patient visits
