Provide virtual primary care services to patients in long-term care and skilled nursing facilities. Manage chronic and acute conditions while supporting quality management initiatives and clinical compliance.
Requires an active NP, PA, or Physician license with a Florida license in good standing. Preference is given to candidates with at least 6 months of experience in long-term care or post-acute settings.
Provide high-quality telehealth primary care services to patients in long-term care and skilled nursing facilities Conduct virtual assessments, diagnose conditions, and develop individualized treatment plans Manage chronic diseases and acute medical conditions through telemedicine platforms Collaborate with interdisciplinary healthcare teams to coordinate patient care Maintain accurate and timely medical documentation in compliance with regulatory standards Participate in quality management (QM) initiatives, including chart audits, compliance reviews, and outcome tracking Ensure adherence to clinical protocols, company policies, and regulatory guidelines Support performance improvement initiatives and promote best practices across care teams Educate patients, caregivers, and facility staff regarding treatment plans and preventative care
Active and unrestricted Nurse Practitioner (NP), Physician Assistant (PA), or Physician license Florida license in good standing required Ability to obtain and maintain additional state licenses as needed Minimum of 6 months of experience in long-term care, skilled nursing, or post-acute care settings preferred Experience with telehealth platforms and electronic medical records (EMR) preferred Strong clinical judgment and patient engagement skills Excellent communication and collaboration abilities Knowledge of regulatory requirements and quality improvement processes within long-term care settings Ability to work efficiently in a fast-paced telehealth environment
