The Social Worker is responsible for developing social histories, creating individualized care plans, and providing emotional support to residents and their families. They also coordinate discharge planning and build relationships with external community partners to ensure holistic care.
Candidates must hold a Bachelor's Degree in Social Work (BSW) or be a certified Social Worker Designee. Experience in long-term care settings and strong interpersonal communication skills are required.
