The Discharge Coordinator plans and manages the discharge process for acutely hospitalized patients. They collaborate with multidisciplinary teams and utilize community resources to ensure a continuum of care.
Candidates must have previous experience in acute care or healthcare and the ability to function in a busy environment. A CA RN or LVN license and knowledge of Medicare, TJC, and NCQA standards are preferred.
Your Role on the Team
The Discharge Coordinator is a professional person who plans, manages and coordinates the discharge plan of the acutely hospitalized patient. The discharge Coordinator works in conjunction with the multidisciplinary team collaborating with the appropriate team members along the continuum of care. The Discharge Coordinator is knowledgeable of available community resources and the healthcare benefits of the acute patient. The Discharge Coordinator will have excellent communication skills both written and verbal and be able to work in an intense environment.
What It Takes to be Successful
Previous acute care, healthcare, or related experience.
One year or above acute care hospital discharge planning experience Working Knowledge of regulatory guidelines including Medicare/CMS guidelines, TJC standards, and NCQA standards. CA RN or LVN license Physical Requirements: Must have dexterity, coordination, visual /auditory acuity to perform all jobs functions. Can sit and or walk for long periods of time. Must be able to function in a busy and distracting environment.
