Salary
$22 - $28 / HOUR
Provide health education and care coordination to help patients meet their healthcare goals and navigate the system. Facilitate group visits and assist patients in accessing community resources and assistance programs.
Requires a high school diploma or GED, with a bachelor's degree in Health/Human Services accepted in lieu of experience. Bilingual fluency in English and Spanish is required, and experience with underserved populations is desired.
Approximately $22.05 - $27.93 per hour. All individual pay rates are calculated based on the candidate's experience and internal equity.
