Provide emotional, social, and grief support to patients and families while navigating complex healthcare processes. Collaborate with interdisciplinary teams to identify social-psychological issues and coordinate special recognition programs.
Requires a Bachelor's or Master's degree in Social Work from a CSWE-accredited program and a valid LSW license. Candidates must have 2 years of medical experience in hospice or palliative care and a reliable vehicle for field visits.

This role is backed by leadership from a dedicated Social Work professional who understands the depth and demands of this work.
