The Choice Case Manager provides intensive case management to families by assessing needs and developing individualized care plans. They facilitate access to community resources, provide crisis intervention, and coordinate with housing departments to ensure participants maintain their housing.
Candidates must hold a Bachelor's or Master's degree in social work, psychology, or a related human services field. A minimum of five years of case management experience, computer proficiency, and reliable transportation are required.
Description The Housing Authority of Kansas City, Missouri is seeking qualified candidates for the position of Choice Case Manager. The Choice Case Manager is responsible for providing intense case management to the families who formerly lived at Chouteau Courts by assessing the family’s needs; developing an individualized plan for each family member; providing resources and referrals as needed; tracking referrals and the outcomes. EDUCATION AND EXPERIENCE Bachelor’s Degree or Master’s degree in social work, psychology or other human services related field. A minimum of five (5) years of case management experience. Computer skills and reliable transportation required.
