The RN Case Manager provides skilled nursing assessments and coordinates care plans to maximize comfort for hospice patients and their families. They collaborate with an interdisciplinary team and maintain detailed electronic medical records while supervising nursing assistants.
Candidates must hold a current VA registered nurse license and have at least two years of professional nursing experience, preferably with one year in hospice. A valid driver's license and a reliable vehicle are required for travel to patient homes.

Ennoble Care is hiring for a Full-time Hospice Registered Nurse Case Manager to work within and around the Loudoun County region in Virginia. As a member of the interdisciplinary team, the RN Case Manager works under the general direction of the Director of Nursing, the attending physician and the interdisciplinary group in providing skilled nursing assessments, planning, and care in order to maximize the comfort and health of both patients and their loved ones/caregivers.
Starting Pay: $90,000
