Salary
$32 / HOUR
Oversee care for high-risk patients transitioning from healthcare facilities and coordinate follow-up visits with primary care teams. Perform clinical tasks including medication preparation, device monitoring, and managing patient data and scheduling.
Must be a graduate of an accredited LPN program with an active Pennsylvania Practical Nursing license and BLS certification. Experience with Medent EHR and the EOS model is preferred.

Meet The Wright Center for Community Health and its affiliated Enterprise entities’ EOS People Analyzer Tool Buy in and experience working in the EOS® model (strongly preferred) Mission-oriented; represents the enterprise in a professional manner while demonstrating organizational pride Graduate of an accredited LPN program Active Pennsylvania Practical Nursing license Previous experience in a health center setting a plus BLS Certification Experience with electronic health record, Medent a plus Ability to work as part of a care team Excellent communication skills Commitment to process improvement and quality *This position requires employees to participate in the following rotation schedule to meet the needs of patients and families: Evenings (one late night per week; 12PM-8PM) Weekends (either one Saturday or Sunday per month; when worked, employees will have a day off during the week) Holidays (one to two holidays per year)
