The APRN provides comprehensive medical care to patients in skilled nursing facilities through routine rounds and assessments. They are responsible for diagnosing conditions, implementing care plans, and collaborating with interdisciplinary teams.
Candidates must hold a current Florida APRN license, National Board Certification, and an active DEA registration. Preference is given to those with at least one year of post-acute experience and bilingual skills in English and Spanish.
The Post-Acute Nurse Practitioner (APRN) provides comprehensive medical care to patients in skilled nursing facilities. This role is responsible for facility rounds, patient assessments, diagnosis and treatment of medical conditions, care coordination, and follow-up management. The APRN collaborates closely with physicians and interdisciplinary teams to ensure high-quality post-acute patient care.
