This position promotes patient and family wellness and manages comprehensive transitional care plans for patients with complex needs. It involves collaboration with service team members, physicians, patients, and families to ensure effective transitions and quality care.
A Master's Degree in Social Work from an accredited program is required, along with at least one year of experience in an acute or post-acute setting. Preferred qualifications include LMSW licensure and case management certifications.
Overview This position would be PRN help on Weekdays Monday - Sunday.
This position strives to promote patient and family wellness, improved care outcomes, access to appropriate hospital and community resources, and manages, supports and develops comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department (ED) environments. Works to promote quality of care and effective transitions planning through collaboration with all service team members, physicians, patients, external agencies and families.
