Provide and coordinate multidisciplinary healthcare services for pediatric patients in their homes, including conducting assessments and implementing individualized care plans. Manage case documentation, supervise home health aides, and collaborate with physicians to ensure cost-efficient and compliant care.
Requires an active New York State RN license and graduation from an accredited nursing program, with a BSN preferred. Candidates should ideally have over one year of experience in CHHA or LTHHCP settings and possess a valid driver's license.
Starting at $105,000/year Key Responsibilities Patient Care & Clinical Oversight Provide and coordinate patient care that is cost-efficient and reimbursable under the patient's insurance plan Conduct initial assessments, case openings, and ongoing evaluations Travel to patient homes or facilities to provide direct nursing care Complete in-person OASIS assessments reflecting accurate clinical and functional patient status Utilize diagnosis coding skills to accurately rate acuity within OASIS documentation Collaborate with physicians and supervisors to determine appropriate primary diagnoses Establish and update individualized plans of care Monitor patient progress and adjust care plans as needed Educate patients and families on disease management and self-care Care Coordination Coordinate services across the multidisciplinary care team (PT, OT, aides, physicians) Participate in weekly PPS meetings for case review and discharge planning Ensure adherence to physician-ordered service frequencies Maintain consistent communication with all care team members Documentation & Compliance Complete 485 physician orders in alignment with insurance authorization and OASIS scoring Document all services, progress notes, and communications in the EHR Maintain accurate records of aide supervision and care plans Ensure timely and compliant documentation Home Health Aide Supervision Supervise aides through home visits Review and maintain aide care plans with staff and families Document weekly supervision Case Management & Program Participation Manage an assigned caseload Facilitate patient admissions and discharges Participate in staff training and orientation Maintain productivity standards On-Call Responsibilities Participate in rotating 24-hour on-call coverage Respond to patient calls and document in the EHR Provide holiday coverage as assigned Additional Responsibilities Support community outreach and referral relationships Complete additional duties and special projects as needed Required Skills & Competencies Strong clinical skills in home health care Knowledge of Medicare PPS and split billing OASIS documentation expertise Diagnosis coding proficiency Strong organizational and care coordination skills EHR and computer proficiency Ability to work both independently and within a team
Active RN license in New York State Graduate of an accredited nursing program (BSN preferred) 1+ year CHHA experience preferred 1+ year LTHHCP experience preferred Valid driver's license and ability to travel
