The LPN Field Staff is responsible for planning and delivering quality home health and hospice care to patients in their homes and facilities. This includes performing ongoing assessments, implementing comprehensive care plans, and ensuring compliance with federal and state regulations.
Candidates must hold a current unencumbered LPN license for the state of Ohio and have at least one year of acute care experience. A valid driver's license and proficiency with Windows and Microsoft Office are also required.
