Provide medically related social services to maintain the physical, mental, and psychosocial well-being of residents. Collaborate with interdisciplinary teams and regulatory agencies to ensure excellent resident care.
Requires a high school diploma, with a preference for a degree in social work or human services and a Social Services Designee Certificate. Previous experience in social services and CNA certification are preferred.
