About the Role

• The Palliative Care Home Visit Nurse Practitioner in Cartersville plays a critical role in delivering compassionate, patient-centered care to individuals with serious, chronic, or life-limiting illnesses within their homes.
• This position focuses on managing symptoms, improving quality of life, and supporting both patients and their families through complex medical and emotional challenges.
• The Nurse Practitioner will collaborate closely with interdisciplinary teams to develop and implement personalized care plans that align with patients' goals and values.
• By providing expert clinical assessments, medication management, and education, this role ensures continuity of care and reduces unnecessary hospitalizations.
• Ultimately, the Nurse Practitioner serves as a trusted healthcare provider who advocates for dignity, comfort, and holistic well-being in the home setting.

Minimum Qualifications

• Current and unrestricted Nurse Practitioner license in the state of Georgia.
• Master’s or Doctoral degree in Nursing from an accredited program.
• Certification as a Nurse Practitioner (e.g., ANCC or AANP) with a focus on adult, gerontology, or family practice.
• Minimum of two years clinical experience in palliative care, hospice, or related healthcare settings.
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
