Responsibilities

Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans Administer medications and monitor patients for side effects or reactions Document any changes in patient conditions and report on changes Maintain accurate records of patient care and discuss observations with the supervisor

Qualifications

• Graduate of an approved school of professional nursing and currently licensed in the state of California.
• Home Health Experience One (1) year Home Health experience .
• Acceptance of philosophy and goals of Agency.
• Ability to exercise initiative and independent judgment.
• Ability to work with individuals to enlist cooperation of many people to perform/achieve a common goal.
