The Nurse Practitioner provides comprehensive diagnosis and individualized treatment plans for patients in the Wound Care Center. Responsibilities include coordinating nursing care, performing physical exams, and collaborating with other healthcare professionals for optimal healing outcomes.
Candidates must be a licensed Advanced Practice Registered Nurse (APRN) in West Virginia with a bachelor's degree in nursing. One to two years of related experience is required, and wound care certification is preferred.
Rivers Health is seeking a full-time Nurse Practitioner for our Wound Care Center.
Provides general medical care and treatment to patients in assigned clinics, emergency room, or urgent care under a collaborative agreement with specified physician(s) as required, by performing the following units.
Responsible to: Assigned Provider and/or Director of Physician Services
Nature of supervision received: Indirect
Assist in the evaluation of clinical personnel assigned.
The following is a summary of the major essential functions of this job. The incumbent may perform other duties, both major and minor, that are not mentioned below; and specific functions may change from time to time.
1. Provides comprehensive diagnosis and wound assessment of patients while managing an individualized would care treatment plan for optimal healing outcomes or chronic maintenance.
1. Coordinate care of assigned patients including nursing care, nutrition, medications, dressing and adjunctive would therapies.
1. Performs physical examinations and preventive health measures within prescribed guidelines.
1. Orders, interprets, and evaluates diagnostic tests to identify and assess patient’s clinical problems and health care needs and collaborate with patient’s primary care provider when comorbidities are contributing to or affecting the healing of a would to determine a comprehensive plan.
1. Records physical findings, and formulates plan and prognosis, based on patient’s condition.
1. Discusses case with Provider and other health professionals to prepare comprehensive patient care plan when appropriate.
1. Submits health care plan and goals of individual patients for periodic review and evaluation by provider.
1. Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy or related therapeutic procedures.
1. Refers patients to provider for consultation or to specialized health resources for treatment.
1. Demonstrates excellence in customer service skills.
1. Fosters an atmosphere of teamwork and collaboration.
1. Participates in quality improvement activities including serving on performance improvement teams.
1. Performs other duties that may be assigned.
1. Complies with Corporate Policies, Departmental Policies and Procedures and Personnel Employee Handbook.
The following qualifications, or equivalents, are the minimum requirements necessary to perform the essential functions of this position.
Licensed, or eligible to be licensed, as an Advanced Practice Registered Nurse (APRN) in the state of West Virginia.
Wound care certification or experience preferred.
Bachelor's degree (B.A.) from a four-year college or university. Successfully completed a bachelor’s degree in nursing. Certified as a Family Nurse Practitioner or specialty as defined.
One to two years related experience and/or training; or equivalent combination of education and experience.
