Case Manager

General nature of position

• Provides case management, to assist mental health, substance abuse and dual diagnosis clients. Assist clients to access medical, educational, social, prevocational, and rehabilitative or other needed community services for eligible individuals. Provides crisis intervention services for clients in various situations. Works with interdisciplinary treatment team to assist the clients in their recovery and management of chronic mental illness, substance use, chronic health disorders and compliance with court requirements, etc. In the spirit of the program, assist the client in accomplishing his/her goals.

• FUNCTIONS: · Provide a safe and welcome environment for people to visit. · Answer phone calls and inquires concerning the program. · Maintain strict confidentiality guidelines regarding all clients, conversations and referrals. · Manages and intensive case load of 15-20 client files · Responsible for performing intake interviews and document data on all residents that reside at said site. · Coordinates services with Clinical Director, Counselor (Interns), team members, primary care and specialist physicians, legal representatives/ guardians, Departments of Mental Health and Alcohol and Drugs staff, probation, parole, pre-trial services, departments of citizen services and other community services/ organizations to assure the individual’s access to services, the services, and the service delivery system. · Assists clients in obtaining housing, financial assistance, health care and other community supports. · Assists clients in completing applications for services, assistance, ID cards, etc. · Provide outreach activities to identify potential clients who have been referred through Drug Court, SAI, Detox Centers, etc. · Assist in developing materials and provide rehabilitation workshops on wellness, health, education, family relationship, relapse prevention, etc. for clients. · Participates in a team environment to provide 24/7 client support · Follows respective documentation standards and documents all activities according to program requirements. · Records services in respective system, maintains client files and completes monthly activity reports. · Performs related work, as required. · Must be willing to work weekends and/or holidays as needed.

Qualifications

Required and Preferred Education and Experience: · High School diploma. · Ability to work with diverse populations. · Good oral and written English skills. · If an individual is in recovery, five-six years of sustained recovery is required. · Computer skills: Microsoft Office and web-based application. · Experience with criminal justice system a plus.
