Coordinate and monitor services to help clients with substance abuse issues achieve stability in the community. Responsibilities include developing treatment plans, conducting assessments, and advocating for clients' basic and specialized needs.
Requires a Bachelor's degree in social work, psychology, or a related field, with a preference for 2 years of experience in chemical dependency. Candidates must pass a drug test and background check and possess a valid driver's license and insurance.

Access, coordinate and monitor services to assist the client in becoming stable in the community and reaching the highest level of potential with alcoholism or substance abuse.
Demonstrate good oral and written communications. Maintain a supportive and encouraging working relationship with co-workers and clients. Participate in the initiation, coordination, and implementation of the referral plan. Follow through, monitor, and evaluate activities related to the recovery of the client. Must have basic computer skills.
Visual acuity and manual dexterity are needed for the required paperwork. Frequent talking and hearing are required when communicating with co-workers, families, and other service providers. Physical demands are light with walking, standing, and occasional stooping.
1. Able to provide customer service to personnel, persons served, families, and stakeholders.
2. Well-maintained health, safety, and welfare of personnel, persons served, and stakeholders.
3. Ensure the agency is operating under CARF accreditation standards.
4. Ensure that state and federal licensure and regulations are being adhered to.
5. Individual assessment of
1. Basic needs such as shelter, food, clothing, entitlements, etc.
2. Specialized needs such as substance abuse treatment services, rehabilitation, social opportunities, vocational and employment capabilities, and psychiatric services.
3. Develop and facilitate life skills learning to help clients prepare for post-program living.
4. Determine clients’ needs for childcare and transportation in order to access services.
6. Send letters to DCBS for clients when they begin working, so they do not lose insurance coverage.
7. Provide ongoing contact with community agencies to develop additional resources and coordinate services.
8. Develop and monitor a treatment plan for each client in the assigned caseload.
9. Advocate for the client.
10. Assist clients with the problems they face in early recovery, especially legal issues, lack of housing, and unemployment.
11. Complete necessary initial documentation with clients including Assurance of Services, Needs Assessment, and Service/Treatment Plan.
12. Complete 5 monthly contacts and document with client- 3 of which must be face to face.
13. Complete Monthly Summary for each client.
14. Maintain a caseload of up to 25 clients.
15. Willing to have on-call responsibilities.
16. Oversee the application process to obtain food stamps for clients.
17. Coordinate with Operations Team on all insurance matters including informing them when a client begins employment.
18. Must maintain TCM credentials by completing 6 CEUs annually.
