In this role, the Care Coordinator will play a critical role in assessing participant needs, developing personalized care plans, and connecting individuals with appropriate resources and services. This position requires a compassionate and organized individual who can collaborate with various service providers, monitor client progress, and advocate for participant needs.
Conduct participant check-ins to monitor overall well-being, progress, and engagement Assist participants with identifying career pathways based on their interests, goals, and strengths Connect participants to employment opportunities, training programs, certifications, and educational resources Provide ongoing support and encouragement throughout the participant’s career development journey Help participants access community resources, including housing, transportation, food assistance, healthcare, mental health services, and other supportive services Support participants with job readiness skills such as resume building, interview preparation, and workplace communication Maintain consistent communication with participants to ensure they remain connected to services and opportunities Track participant progress, goals, and service needs through case notes and documentation systems Coordinate referrals and warm handoffs to partner organizations and external service providers Assist participants with overcoming barriers that may impact employment, training, or personal stability
Previous experience as a Care Coordinator or in a similar position is preferred Certification as a medical assistant is preferred Strong problem-solving and organizational skills Ability to manage multiple projects or tasks and prioritize appropriately Ability to work in fast-paced situations and make sound decisions quickly Excellent interpersonal skills and high level of compassion Strong verbal and written communication skills
