The Social Worker supports patients and families in adjusting to center life and managing crises related to illness, disability, and death. Key duties include coordinating discharge planning, advocating for patient grievances, and collaborating on multidisciplinary care plans.
Applicants must hold a Bachelor's or Master's degree in social work from an accredited program. Knowledge of the case management process and the specific needs of geriatric patients, including funding resources, is required.
