Now for the fine print

This is a 40 hour position located in Wilmington, MA directly on the commuter rail. One weekend day is required. Previous experience with substance use disorder treatment and case management REQUIRED.

• Our Case Managers at Mayflower Recovery are responsible for providing confidential addiction counseling.
• Our Case Management team provides education and support to individuals, families, and the community.
• Completes all required clinical documentation for their caseload.
• Promotes healthy lifestyles and healthy choices.
• Under the supervision of the Clinical Supervisor, and in collaboration with a multidisciplinary treatment team, the Case Manager ensures post treatment placement to the most appropriate level of care as recommended for each client.

Specific areas of responsibility to position

• Complete all clinical documentation for caseload including Biopsychosocial assessment, treatment plans, individual progress notes and collateral contact with supports and aftercare sources.
• Developing the patient's discharge/transfer plans to ensure movement to the appropriate levels of care.
• Contacting referral sources, providing case consultation and coordination with referral sources.
• Coordinating ongoing care for housing and therapeutic needs with the Treatment Team, substance abuse treatment providers, healthcare providers, community resources, and family members.
