Coordinate care and perform continuous assessments of patient and family needs to implement interdisciplinary care plans. Ensure all services are delivered according to physician orders and comply with federal and state regulations.
Candidates must be a graduate of an approved clinical education school, hold a valid state license, and have at least one year of professional experience. A valid driver's license, automobile insurance, and CPR certification are also required.
Provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan. Perform services in accordance with the physician’s orders and the established plan of care (POC). Ensure delivery of care is compliant with the federal and state regulations.
Must be a graduate of an approved school of clinical education. Must be licensed in the state where they currently practice. Must have at least one year experience as a licensed professional. Must have basic demonstrated technology skills, including operation of a mobile device. Education and experience, preferred Experience as a licensed professional may be deferred with a review and approval from the regional and executive service line clinical leadership. Medicare home health or hospice experience is preferred.
