Lowndes case management - Case manager

• We are seeking a Case Manager to work in Valdosta, Georgia.
• The Case Manager, under general supervision performs a wide range of tasks in the community to assist consumers in regaining control over their own lives while promoting the recovery process.
• Utilizing psychiatric rehabilitation interventions provides skills training, education and case management to consumers and their families.
• Monitors consumer progress towards treatment goals.
• Extensive documentation is required; accurately charts services and consumer progress.
• Must be able to work well with community agencies and function as part of a team.
• This position requires: Bachelor’s degree in social work, psychology, mental health, nursing, education, sociology, criminal justice, counseling, rehabilitation counseling, guidance counseling or an equivalent field AND one year of full-time experience in social services delivery.
• Knowledge of mental illness Valid Georgia driver’s license

Requires varying hours of duty to provide monitoring, educational and/or crisis handling, as well as travel to off campus locations in LBHS or personal vehicle.
