The Intensive Housing & Health Case Manager is a vital member of the Supportive Housing Program (SHP) team, delivering comprehensive, individualized support to participants using a client-centered, harm reduction, and trauma-informed approach. This role blends hands-on housing case management with health navigation, helping participants achieve and maintain stability in both their housing and healthcare.
This position involves regular home visits, accompanying participants to medical and benefits appointments, and connecting them to community-based resources — all while ensuring full compliance with funder documentation requirements, including the Illinois Department of Human Services (IDHS) and Housing Opportunities for People with AIDS (HOPWA).

This is a hybrid role that includes both field-based and office work. Field responsibilities involve regular travel throughout Chicago to conduct home visits and accompany participants to appointments. Office tasks may include extended time at a workstation. Occasional light lifting (up to 10 lbs) may be required.
