Primary Responsibilities Responsible for the diagnosis and treatment of acute, chronic, and long-term healthcare issues, in collaboration with patient’s Primary Care Physician Effectively identifies, evaluates, and addresses disease prevention and health promotion issues while administering quality patient care. Maintains accurate medical records and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals and consultations. Educates patients and/or families about preventative care, medical issues, and use of prescribed medical treatments and/or medications.

Advanced Nurse Practitioner Certification in Primary Care, Geriatric, Adult, with unrestricted licensure in the state of NY at time of employment Current and unrestricted DEA certificate required or ability to obtain Valid driver’s license Ownership of a reliable and insured vehicle, with the ability to travel within assigned territory to meet with patients Comfortable working independently in a home-visit setting and clinic setting Ability to communicate with patients, primary care team members and management staff Ability to make care decisions based on best medical practices without direct supervision or guidance by a physician Basic technical proficiency in the use of computer programs for medical records or internet search for clinical practice Advanced assessment and treatment plan implementation skills
Effective communication skills Effective organization skills and ability to multi-task Takes Initiative, creative, has problem solving ability, is adaptable, and flexible Ability to work without direct supervision and practice autonomously Geriatric care experience Knowledge of mental health issues 1+ years of Outpatient Clinic experience Familiarity with Medicare and ICD-10 coding Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
