Hospice Intake/Admissions Coordinator The Hospice Intake/Admissions Coordinator is responsible for managing the intake and admissions process for all hospice referrals. This role serves as the first point of contact for referral sources, patients, and families, ensuring timely and accurate processing of referrals and facilitating a smooth transition into hospice care. The coordinator will gather and verify clinical and insurance information, communicate with physicians and healthcare facilities, and collaborate closely with the admissions team, nursing staff, and other departments to ensure a seamless and compassionate admissions experience.
