Clare Hospital in Baraboo working a total of 40 hour per week.
Work Schedule Weekdays: Shifts can start as early as 6:30 AM and end as late as 5:00 PM or earlier.
Weekend shifts: 6:30 AM–3:00 PM. A one-time sign on bonus (less taxes) may be available to qualified individuals with a two-year service commitment.

Requirements: PRIMARY RESPONSIBILITIES Ensures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring and dose/therapy adjustment as applicable. Documents medication related plans and interventions in electronic medical record. Provides clinical consultation delivering best-practice, evidence-based and complete information to practitioners. Makes recommendations and encourages safe and cost-effective pharmacotherapy for patients through multi-disciplinary collaboration. Oversees the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy to patients. This includes supervision of pharmacy technicians and/or interns who support the role of the pharmacist in the drug delivery system. Precept pharmacy students and residents, as applicable. Works in a constant state of alertness and safe manner. Performs other duties as assigned. EDUCATION Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from accredited school of pharmacy EXPERIENCE No experience required PHYSICAL REQUIREMENTS Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs. Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements. Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors. Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc. Frequent keyboard use/data entry.
