The LPN supports providers by recording vital signs, performing tests, and delivering nursing care in collaboration with RNs and Physicians. Responsibilities include maintaining medical records, administering injections, and providing emotional support to patients.
Candidates must hold a NYS LPN license, BLS certification, and a high school diploma or GED. At least one year of experience working with clients with Substance Use Disorder or Serious Mental Illness is required.
What You Will Do Assists with patient care including checking vital signs, blood pressure, pulse rate, breathing, temperature, weight, discussing chief health concerns, administers injections and performs test. Takes patient’s medical history, all pretreatment formalities and enters data in computer system. Follows orders of physician and registered nurse in providing care. Records in patients’ record a detailed report of care given. Handles proper charting, documentation and maintenance of patients’ medical records. Checks, cleans and dresses wounds. Reinforces instructions given to patient by RN or Physician. Notifies RN or Physician of significant finding or changes in patient’s physical or mental condition. Places phone calls to pharmacies to order prescriptions. Maintains confidentiality of patient records. Pulls and prepares patient charts. Maintains neatness of work area. Stocks patient rooms with supplies. Provides emotional support as needed. Initiates emergency assistance as per policies and procedures. Attends staff development trainings and nursing conferences. Keeps skills current and increases knowledge and productivity. Performs other duties as requested.
Who You Will Be NYS LPN licensure. BLS certification. High School or GED. Graduate of a formal Technical or Vocational LPN Program. At least One (1) year experience as an LPN working with clients with Substance Use Disorder (SUD) and/or Serious Mental Illness (SMI). Available to work a flexible schedule as needed in response to participant and staff needs. Computer literacy including proficiency in Microsoft Office Suite and EHR. Ability to put patients at ease and explain physician’s instructions. Ability to multi-task with excellent organization skills. Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds. Knowledge of medical terminology. Team player and ability to work independently. #li-onsite
