Opportunity
Details: This position encompasses work of varying degrees of difficulty in a multi-faceted, CDC-funded program that aims to reduce HIV and other communicable diseases. Ranging from basic case management to planning, organizing, and conducting of disease control efforts in these areas, and requires data analysis to support programmatic quality improvement efforts under the direction of physician supervision. Identification of candidates for routine HIV testing in adult Emergency Department. Interact with clients to perform and document rapid, point-of-care HIV testing in accordance with hospital, local, and state requirements and guidelines. Provide clients receiving rapid, point-of-care tests, and give appropriate pre- and post-counseling and education. Responsible for linking newly diagnosed HIV+ to care in the Jacobi or North Central Bronx HIV clinics for immediate antiviral treatment. Responsible for linking eligible patients to the appropriate clinical outpatient care. This includes clients with newly diagnosed HIV+, out of care HIV+, sexually transmitted diseases, and clients interested in PrEP and/or for follow-up care for PEP that has been initiated. Provide counseling and PrEP/PEP education for HIV- clients. Responsible for documentation in the EMR of patient interactions and communications per program protocols. Participation in focus groups and individual interviewing of patients for program enhancement. Attend job related education training, seminars, and conferences as determined by the program manager.
