The Hospice LVN provides direct hands-on care and ensures the safe delivery of hospice services to promote patient comfort and quality of life. They are responsible for obtaining orders and keeping the Case Manager informed of any changes in patient condition.
Candidates must hold a current LVN license in the State of California and have at least one year of acute-care experience. Experience in hospice and proficiency with EMR systems are preferred.

The Hospice LVN position is open in our Encino location and surrounding areas. This is a Full-time position during our evening shift from 3:30pm-midnight.
of the Hospice LVN: · Current LVN license in State of CA · Minimum one year of acute-care experience · Experience in hospice preferred/EMR experience preferred
· Ensures quality and safe delivery of Hospice services · Implements current nursing practices and provide direct hands-on care within scope of practice · Obtains orders as needed to promote comfort and quality of life · Keep Case Manager informed of all changes in condition and orders obtained
