The LPN is responsible for identifying resident needs, implementing prescribed interventions, and evaluating health outcomes. They provide personal care and collaborate with the care team to enhance delivery and participate in quality improvement activities.
Applicants must be graduates of a School of Nursing for LPN/LVNs with current CPR, AED, and First Aid certifications. Preference is given to those with at least 6 months of experience and a background in geriatric care.

Licensed Practical Nurse - LPN As a LPN you will be responsible for delivering care and services to residents through the identification of resident needs and implementing prescribed interventions along with evaluating outcomes and conditions. This position performs personal care for residents as needed and communicates and collaborates with other members of the care team to resolve resident care problems, enhance care delivery and participate in quality assessment and performance improvement activities.
